Our Community Support Services (CSS) program empowers adults with severe and persistent mental health conditions to overcome social and systemic barriers and integrate meaningfully into their communities.

Through personalized rehabilitation plans and a wellness-based approach, CSS focuses on developing life skills, maintaining stable housing, accessing resources, and fostering independence. Case managers work collaboratively with each individual to identify goals and build supports aligned with the eight dimensions of wellness: emotional, environmental, intellectual, physical, occupational, spiritual, social, and financial.

Daily Skills

Assistance with tasks like budgeting, grocery shopping, nutrition, housekeeping, and personal hygiene to support everyday independence.

Community Integration

Support in building healthy neighbor relationships, understanding tenant rights, and developing a positive presence in the community.

Case Management

Individualized support plans and referrals to services, benefits, and resources to ensure access to the care and opportunities you need.

Wellness Planning

Collaborative goal-setting focused on the eight dimensions of wellness to support long-term recovery, growth, and self-sufficiency.

Program Eligibility Requirements

Community Support Services are available to adults (18+) residing in Monmouth County who have been diagnosed with a severe and persistent mental health condition. Referrals may come from community providers, hospitals, or individuals themselves. Services are offered at no cost to participants.
