  • What does direct access to physical therapy mean to you?

We get the question all the time….”Do I need to get a prescription from my doctor to come see you?” The answer is NO except for one situation – if you sustained an injury in a motor vehicle accident and you are using your auto insurance for payment of services. In that case, physician referral is required.

Here are some examples of conditions for which our patients come directly to physical therapy for immediate treatment:

    1. I woke up in the morning with a stiff neck and I can’t turn my head without pain.
    2. My son injured his knee during a soccer game and I can’t sit in an ER for hours.
    3. My husband lifted a box in the garage and now he can’t straighten up.
    4. I sprained my ankle during a basketball game and I have swelling.
    5. My daughter was hit in the head during a soccer game and now has headaches and trouble concentrating at school.

If you have an injury or pain syndrome that does not require a physician to evaluate, don’t wait to start physical therapy. Besides the cost savings, there may also be a delay in getting treatment which can slow your recovery.

How do I choose the right physical therapist?

The key to this question is to choose the physical therapist, not the physical therapy practice. You are developing an important relationship with a healthcare provider that you may be with for an extended period of time to recover from an injury or after surgery. People are quite often referred to physical therapy for a consultation after seeing their physician or surgeon. What should you consider when choosing the right provider?

You need to know the credentials of the person that will be performing an evaluation and subsequent treatment. Do they have experience in your problem area? Do they have advanced skills or certifications? Will they be the only person treating you throughout the course of your recovery? When being referred to a practice but not a specific physical therapist, you risk getting anyone that is available. They may not have the experience level, advanced clinical skills or ownership in the facility. That is the most important trait since practice owners will go above and beyond in helping you recover.

Have you ever been to a dentist, veterinarian, podiatrist, or a physician that was not the owner of their practice? Unfortunately, the profession of physical therapy has been ravaged by corporations or physicians that self-refer to a practice they own as profit centers. Most of those practices work under the guise of high volume and sharing of patients between physical therapists. Is that the best environment for you to recover?

You have a choice when seeing a physical therapist. Choose wisely and remember, you always have a choice to go wherever you prefer. Get a recommendation from a friend or family member and always choose based on the credentials of the specific physical therapist that will be treating you. Can you learn about the physical therapists on the practice website? If not, the practice probably doesn’t want you to know about each therapist but instead know only about the practice name. db Orthopedic Physical Therapy’s two locations are only staffed by partners in the practice with advanced clinical skills. Keep us in mind for any of your rehabilitation needs.

My child has a concussion – now what do I do?

The incidence of concussion in youth sports continues to rise, and parents are often left with trying to navigate the process on their own.

The highest incidence rate occurs in football, boys’ ice hockey, girls’ soccer and boys’ lacrosse but other sports are not immune. Unfortunately most parents get their first piece of education about the concussion process in the local emergency room which is not always the best place to go following a mild head injury. The ER is often loud, bright and hectic, which is overstimulating to the concussed athlete, and can make the symptoms of headache, dizziness and irritability even worse.

In the far majority of situations after a mild head injury or concussion, quiet rest is the most important treatment. This allows the brain to heal during a vulnerable time.

Our internal system that regulates blood flow to the brain gets impaired during a concussion, so when the brain needs increased blood flow to provide healing nutrients, it actually gets less. If the brain is stressed during this healing time with concentration tasks, memory functions, visual and auditory processing and exercise or activity, the healing process gets interrupted which can delay the recovery process. The key is to get your child to rest to allow them to avoid any stimulating activities. Sleep is good and will improve healing.

    Post Concussion Treatment

Oftentimes, following a concussion, a supervised Concussion Rehabilitation

program is recommended. At db Orthopedic Physical Therapy, Dr. Bertone provides a 5-step comprehensive post-concussion therapy program to improve balance, regain postural control, reduce headaches and improved cognitive function. As progress is made, Dr. Bertone continues

the graduated program with more advanced training, all intended to encourage the safe return to physical activity and sports competition.

    Baseline concussion testing

ConcussionRXcare at db Orthopedic Physical Therapy offers comprehensive baseline concussion testing to determine a pre-injury status of your balance, neurocognitive and oculomotor function. This information is stored and used for comparison purposes in case the athlete sustains a head injury. If you suffer a concussion and return to physical activity before you are fully healed, you are at high risk for the potentially tragic “Second Impact Syndrome.” This is particularly important with athletes who play sports where concussion is a common injury.

The only way to determine when it is truly safe to return to physical activity is to test your brain function before you have a concussion. To get the most accurate result, baseline testing must be comprehensive and the results interpreted by an expert in the field. This information is then sent to your physician/pediatrician with a report to help them make the proper

return to play decision. If the results are impaired compared to the baseline levels, the athlete is retested until they return to prior levels.

I had 3 spinal surgeries in the past 2 years and been to numerous physical therapists and I could say without a doubt Dr. Jerry is the only one that has really helped me. His concern, consideration and knowledge surpasses all the others that I have dealt with in the past. I only wish that I found Jerry sooner. I highly recommend him to anyone in need of PT.

“db Orthopedic Physical Therapy really helped me with my lower back pain! With Dr. Bertone’s help I feel like I have more mobility and strength and no more back spasms. He and his staff were very professional, friendly and always on time!! You feel like you are really being cared for in this practice! I would highly recommend Dr. Bertone to anyone in need of Physical Therapy either for Rehab after surgery or like in my case a chronic condition that can be treated conservatively without injections or surgery. Thank you again!”
